When your child is ready to leave the hospital, we will help you make plans to leave the hospital. We want your transition to be as easy as possible.

When can we go home?

  • After your child's main doctor has approved your child for discharge

Who will help us plan our return home?

  • Your entire health care team develops a discharge plan to meet your family’s unique needs and ensure proper care for your child
  • Case managers (registered nurses) also help coordinate care

What do case managers do? They...

  • Work with your child's entire health care team
  • Talk with you about what you need to take care of your child at home
  • Arrange for special equipment and home care services, if needed

Who do I ask about insurance and billing questions?

  • Contact your insurance company directly about coverage questions
  • Learn more about billing and understanding your bill
  • Visit or call (773.880.4273) the hospital’s Business Office

Before we go home, what should we do?

  • Ask questions – we want to address any concerns you have about caring for your child
  • Have a transportation plan ready
  • Understand all instructions about your child's medications, diet, special equipment, follow-up care, home health or supportive services
