Bone Marrow Failure in Children and Adolescents by Seth J. Corey, MD, MPH Published Fall 2010

Pediatricians often encounter single or multiple cytopenias and must diagnose the cause expeditiously and accurately, since cytopenias may be self-limiting or chronic, benign or malignant. One of the serious causes of cytopenia to rule out is a bone marrow failure syndrome, which may be inherited or acquired. Because some of the inherited bone marrow failure syndromes are associated with cancer predisposition, surveillance is critical and management complex. As these syndromes are often difficult to diagnose, this article will review the features that will help pediatricians differentiate potential bone marrow failure syndromes from other causes of cytopenia(s), as well as discuss the additional testing required to confirm specific disorders. To help pediatricians counsel families, the article also will present a general approach to management of bone marrow failure disorders in specialty care and options for genetic counseling.
