The first appointment

Here is information to help you prepare for your child's first appointment, as well as some general tips from other parents
on how to best prepare for a hospital visit.

Before your visit:

  • Register online using a secure form, or by calling 1.800.KIDS DOC.

  • Gather all the necessary documents to bring, including:

    • Your child's medical records, including radiographic imaging reports and actual images (including CT and MRIs) on a CD, surgery reports, pathology report from initial and most recent surgery and a detailed description of past treatment. 
    • For patients participating in a clinical trial, two copies of your child’s most recent MRIs.
    • Brain tumor coordinator, Wendy Stellpflug, discusses her role in assisting patients and families through the treatment process. Watch video »

      A list of your child's current medications, including the name of medicine, the dose and frequency.
    • Your insurance information.
  • Contact your insurance providers, primary care physician and your primary oncologist or doctor currently managing your care to let them know you have an appointment with us.

  • Find out more about clinical trials and what to expect about treatment.

  • Learn more about Family Services, trained professionals who help connect families with resources, support, and more.

  • We ask that all families be prepared to stay in Chicago at least one week. Coming in from out of town?  Find local hotels that often offer a discount to our patient families. 

  • The hospital may be able to provide transportation to and from the hospital and three hotels (Belden-Stratford, Days Inn - Lincoln Park North, and Comfort Inn). Call Children's Memorial Security at 773.880.4222 to learn more.
  • The Ronald McDonald House often provides lodging for Children's Memorial patient families.
