Your child's first appointment
Thank you for choosing us for your child's healthcare needs. Here are some suggestions to help you prepare, and ensure your child receives the best possible care.
Before the visit
Bring with you
- Copies of any previous test results (x-rays, CT scans, MRIs, blood work).
- All medications your child is currently taking. Also, know you can continue to give your child any previously prescribed medications.
- Your insurance card. Also, know all insurance co-payments are collected at the time of service.
- Because all Chronic Pain Program patients receive a psychological evaluation, you may want to contact your insurance carrier to understand your plan's specific mental health coverage. If your plan requires a referral, you can fax it to 773.880.3331 or bring it with you.

About psychological evaluations: Since chronic pain often causes disruption in normal childhood activities, all Chronic Pain Program patients receive psychological evaluations. Our pediatric psychologist can recommend ways to strengthen coping skills and suggest ways to deal with pain. Please note, you will receive a separate bill for this evaluation.

During the visit
- Be prepared to spend up to 3 hours with us.
- Know that your child, in addition to being seen by an attending physician, nurse practitioners and medical psychologist, may be seen by pediatric residents and fellows.
